MOTI is a Medicare provider, and we also accept all PPO insurances. For some PPO plans, MOTI is an in-network provider, and for others MOTI is an out of network provider. For those insurers for which we are out of network, MOTI can provide a super-bill to send to your insurance for your reimbursement.<\/p>\n
MOTI is currently in network with Medicare and most PPO plans from Anthem, Blue Shield of California, Aetna, HealthNet, Tricare West, and TriWest. You should always confirm coverage directly with your insurance carrier, and ask them specifically about physical therapy benefits at MOTI Physiotherapy, Inc.<\/p>\n
Most insurance companies currently require a physician\u2019s prescription for physical therapy in order to cover services. Should you need to obtain a physician\u2019s prescription for physical therapy, our office can help you find an appropriate referral source.<\/p>\n
MOTI Physiotherapy accepts patients without a physician referral. In the state of California, patients are able to be evaluated and treated by licensed physical therapists without a physician referral under the Direct Access legislation. This means you no longer have to wait to get treated for injuries that would immediately benefit from physical therapy services. Under California law, patients may receive treatment from a licensed physical therapist for 12 visits or up to 45 days without a physician referral or prescription.<\/p>\n
This allows patients to expedite treatment while saving money. Expedited treatment time helps reduce total injury time and allows you return to normal function faster. It saves you from having a costly doctor visit for a prescription prior to a physical therapy visit.<\/p>\n
For self-pay patients and some insurances, you do not need a physician referral to come to MOTI for treatment. However, many insurances still require a physician referral (that is not more than 90 days old) in order to cover your PT visits.<\/p>\n
When you schedule an initial Physical Therapy appointment at MOTI (and you wish to bill your treatments through your insurance), please provide your insurance plan information. Here are explanations of three important insurance terms that represent the portion of the medical bill that you are responsible for.<\/p>\n
Deductible –\u00a0<\/strong>A deductible is usually a fix dollar amount that you have to pay out of your own pocket before the insurance will cover the remaining eligible expenses. Depending on the insurance plan, the deductible can range from $0 all the way up to thousands of dollars. It can also be paid per sickness\/injury (per condition) or per certificate period.<\/p>\n Coinsurance –\u00a0<\/strong>Coinsurance is usually a percentage, and represents the percentage cost that you will need to pay and the insurance plan will pay towards your eligible medical expenses. Some common coinsurance examples include: 80\/20, 90\/10 and 50\/50 \u2013 so if you have 80\/20 coinsurance on your insurance plan, it means that the insurance company will cover 80% of your medical cost and you are responsible for paying the other 20% yourself.\u00a0A deductible is commonly use together with coinsurance. In this case you would pay the deductible amount first and after you would have the left over coinsurance amount.<\/p>\n Copay (copayment) –\u00a0<\/strong>Copays are similar to deductibles, in that it is usually a fixed amount of money you have to pay each time you need to use your insurance plan. Unlike deductibles, copays tend to be smaller dollar amounts and are applied on a per visit basis so that that you would have to pay it each visit.
